Life vest defibrillators are external devices that can be worn to reset patients' hearts by DC cardio version if they develop a life threatening rhythm - VF.  The devices are expensive to use and not tolerated by all patients for a variety of psychologic reasons.  The article summarises the early experience at Westmead Hospital with this devie and reviews the potential indications along with the economic impacts /costs for these decisions

Pacemaker implantation is an established therapy for patients with syncope and bradycardia.  Like all technologies it has potential benefits and risks and a cost to the patient and community.  This article reviews the indications for pacing in patients. It looks beyond the obvious patient with bradycardia and recurrent syncope to include those predicted to be at high risk and clarifies situations where pacing is unlikely to be of benefit

Patients with atrial fibrillation are at increased risk for thromboembolism.  This article reviews the use of appendage closure devices for treating patients who are not able to have long term anticoagulation
